To ensure a smooth and efficient appointment, we kindly ask that you gather the following items and bring them with you: 

Your insurance card and photo ID

• Any medications, vitamins, supplements, or injections you are currently taking
• The name, address, and phone number of your pharmacy
• The names of all specialists you see, including eye doctors, cardiologists, podiatrists, endocrinologists, neurologists, or any other
• The name of any home health agency that provides service to you in-home
• The names of any medical equipment supply companies you use, such as your oxygen supplier, pacemaker manufacturer for device monitoring, diabetic pump supplier, or any other device that is important to maintaining your health
• A list of any recent immunizations you have received such as Tetanus, Hepatitis B, Pneumonia, Flu, Covid vaccine, Herpes Zoster (shingles), or any other
